This patient story reveals how our little patient fell victim to early childhood caries due to the lapse in regular dental examinations. Chief Complaint

Siya approached us with a chief complaint of noticing black lines on her upper front teeth. During her visit a year ago, she was cavity-free. Regrettably, her second visit, a year and a few months later, revealed the development of new cavities. A thorough examination uncovered multiple initial carious lesions between her upper anterior teeth. Now, our goal is to address these concerns and ensure Siya’s continued oral health and satisfaction with our dental care.

Diagnosis / Medical Assessment

We conducted diagnostic radiographs to accurately evaluate the extent of the carious lesion, which is crucial for proper diagnosis and treatment planning. An increase in the extent of the carious lesion may require pulp therapy. Fortunately, the radiographs confirmed that Siya did not require pulp therapy. However, Siya’s teeth were affected by the early initiation of early childhood caries. Before proceeding with treatment, we took the time to explain to the parents the factors contributing to the development of cavities, aiming to prevent future occurrences and promote better oral health habits.

Treatment

Our holistic approach to managing early childhood caries is divided into two parts. Firstly, parents implemented home hygiene measures to decrease the bacterial load in the child’s mouth, preventing further occurrences of cavities. Secondly, a minimally invasive treatment plan is tailored to address the present condition. .

Following the treatment plan, comprehensive oral prophylaxis was performed, and silver diamine fluoride (SDF) was applied to address proximal carious lesions. The SDF application was conducted without drilling, aimed at halting the cavity’s progression, eliminating cavity-causing microbes, and discouraging further spread of the caries. SDF is non-invasive, painless, and comfortable for the little one.

The treatment followed with fluoride varnish application to prevent new cavities. Fluoride in the varnish penetrates the tooth enamel, strengthening the tooth. Parents received instructions regarding diet modification and oral hygiene.
